I Enter restaurant name * , hereby agrees to provide the following accurate information and agree by the terms provided herein in order to become part of marketing of the Restaurant by Foodie Card, Inc. ("Foodie"): If the Foodie Card is presented at your Restaurant, you agree to provide the discount offered to Foodie members, to wit, providing a 10% discount to the Foodie customers. Restaurants reserve the right to refuse a Foodie Card on the following holidays: Christmas Eve, Easter, Father's Day, Mother's Day, New Year's Eve, Thanksgiving Eve, Valentine's Day. Restaurant reserves the right to refuse Foodie Card customers as part of another coupon or promotion.

Foodie makes no representations as to whether additional business will be generated by virtue of applying with Foodie. The Restaurant agrees that Foodie is not liable for any of the food, drinks, products or other services provided by the Restaurant and the Restaurants agrees to indemnify and hold harmless Foodie should any claims arise out of a customer's experience with the Restaurant. The Restaurant will provide a minimum of thirty (30) day’s notice upon cancellation of such terms.
